  2. In the form below, fill out the top section with your information.
  3. In the next section, ONLY fill out the information that needs to be changed in our directory.  

After you fill out this form, we will send you a confirmation email.  Follow the instructions in the email.

All change requests are subject to approval by the club leadership.
